Acrometastasis of Hand (A case of metastasis in five distal phalanges from an adenocarcinoma of stomach)

Abstract
Although osseous metastases have been discovered in approximately 30% of all patients at terminal stage with cancer, only 0.007 to 0.3% of such patients have acrometastases in the hands and feet. Acrometastasis occurs as a rare pre-terminal event and often is a part of a widespread dissemination of metastasis. We experienced a very unusual case of acrometastasis in all fingers of the left hand of a 67-year-old female, who was diagnosed as a gastric adenocarcinoma 4 years ago which metastasized to the liver, lung, and left humerus. Our final treatment for the acrometastasis was a shoulder disarticulation for the relief of pain. Perhaps the increased blood flow of the humerus with pathologic fracture was responsible for the localization of the tumor emboli in all distal phalanges of the hand on the ipsilateral side. The length of survival in this patient with the acrometastasis from the adenocarcinoma of stomach was only 15 weeks.
